On behalf of all of our representatives, employees and 1000+ winners, Congratulations on your great accomplishment. In achieving perfection you are entitled to your winning claim of one of the many fabulous Hole-in-WON.com Awards. Please understand that there is a minor process still necessary to complete your claim. This document will help you to understand how to complete these compulsory steps. This process is generally painless and simple, but nonetheless, a necessary, important, and imperative part of Validating your Prize and Contest Claim.

Step 1.An internal verification of your event contract, terms, conditions, and circumstances are checked to make sure all obligations have been met, and that VALID PAYMENT has been made. (Checks must be cleared, Credit cards processed, and Bank wires validated). ALL Past Due Accounts Settled. NO Exceptions. NOTE: With recent banking regulations this can take as much as 10-20 business days for checks, 5-10 business days for Credit Cards, and 1-5 business days for bank wires ). International Accounts are subject to each country banking jurisdiction and rules. ALL Events personnel, witness(es), and winners are subject to ALL terms, ALL conditions, and ALL stipulations on ALL Contracts, Addendums, Specific Rules, Eligibility Confirmation of Terms, Attestment Forms, Video Requirements, and supplementary award HIO restrictions. Receipt of this document is NOT a Waiver or Release to ANY other document(s), term(s) and conditions(s), NOR is this an acceptance of those terms and conditions in regard to your prize claim, NOR an acceptance of the validity of your prize claim at this time.

Step 2.Attestment forms for completion by the winner, client, tournament director, and witness(es) will be EMAILED out within 48 Hrs. after Step 1 is Complete. AND this Congratulations form is Signed and Dated by the potential winner with Contract Contact person. IMPORTANT: You, the winner, tournament director, assignee and/or client are 100% responsible for the proper timely completion AND proper delivery of these forms. Each form must be completed and signed by the appropriate person, witness(es) only and individually. There is to be NO assistance of any kind by a second party in the completion or partial completion of any form.

A) Hole-in-WON.com is in NO way responsible for the monitoring of proper completion, timely completion and final completion of these forms, NOR accepts responsibility for delays in delivery, incomplete forms, missing forms, lost mail, improper Videotape or unsupported video formats, NOR checking on the status of completion of these attestment forms and/or Videotape(s) AT ANY TIME. Again you the client, winner and your assignees are 100% responsible for the timely proper completion AND delivery of the necessary Attestment forms and Video requirements.

B)All forms, scorecard, unedited videotape and tournament contestant lists must be sent together. Otherwise they will NOT be accepted. Faxes are NOT allowed at any time. No Exceptions. Overnight express is not necessary. Registered mail receipt is only recommended, NOT required.

C)You have until Dec. 15^th of the same year of your winning claim to have all attestment forms completed and they must ALSO be received in the offices of Hole-in-WON.com, Rye NY, by Dec. 21^st. All claims after these dates will be deemed null and void and invalid.

D) Hole-in-WON.com is contractually obligated to respond and fulfill prize award for and to Contract signee ONLY. Should a release or replace of that person be required, please contact Hole-in-WON.com for that assignment.

Step 2A.The required documentation, forms, and video evidence will be determined solely by a Hole-in-WON.com official in conjunction with the requirements of the sports contest, $$-Value of prize, type of prize award (cash, trip, auto, supplementary (non-main), PLUS the type of sports contest event and Hole-in-WON sponsor(s), promotional, 3rd party, contest sponsor, client, other contest performance or prize selection fulfillment. As per the conditions set forth here in and as the specific sports contest(s), contract(s), rule(s), addendum(s), award choice(s), terms, and conditions set forth herein.

To Include Completion of:

a) Winning Contestant Attestment Form

b) Event Director Attestment Form.

c)  Independent 1 and 2 Witness(es) Attestment Form(s) for Event

d) Independent 1 and 2 Witness(es) Attestment Form(s) for Selection of Contestant(s) Process.

e)  Independent 1 and 2 Witness(es) Attestment Form(s) for Rules Distance Sport Conforming.

f)  Other Competitor(s) Attestment Form(s).

g) Unedited Video Tape(s)

h) Sporting Event Entry Form and Entry Sheets (shows events official participants).

i)   Diagram of Sport Contest Event (for ease of verification)

j)  W-9 Tax Requirement.

k) Any additional witness as per request Hole-in-WON officials.

In addition, Attestment Forms may include:

a)  Sport Professional and/or Police Official Attestment Form(s)

b)  Any additional forms as per request of Hole-in-WON.com

c)  Additional video tape(s)

d)  Rules Conforming witness(es).

NOTE:These are simple 1 page documents that are easily completed in minutes and results in a quick, thorough verification that eliminates fraud, which in turn keeps rates stable for other events, charity(s) enjoyment worldwide.

Step 3.Once Hole-in-WON.com receives attestment forms a verbal verification is performed with all attestment form applicants. Review of the unedited videotape that needs to determine without question proper timing, rules, distances and all other necessary stipulations of the Sports Contest contractual agreement, addendum(s), rules, and conditions. Due to circumstances that may occur beyond our control during this verbal verification, we can make NO CLAIM as to precise or approximate turnaround of this phase of the validation. Exercise patience. Once completed your claim is NOW valid and complete. Note: (Delays WILL occur during the height of golf season, March-Oct.) and our claim offices are closed the last two weeks in November through the first week of December, all holidays, and two-plus weeks during the year ending Christmas, New Year Holidays annually. Hole-in-WON.com will NOT, nor can we accelerate this final claim process in order to offset slowness, delays, or errors in completion or delivery of the forms. Hole-in-WON.com has the right to request a polygraph test should they deem it necessary.

a) Once you have won a VERIFIED award:Payment will be then put into our accounts payable department, and you will receive your check within 5-10 business days from the day our accounts payable processes it. PAYPAL may be available upon request. (Delays may occur during the height of golf season, March-Oct.). Installment plans for any prize, any time, and any amount is the SOLE discretion of Hole-in-WON. NO Exceptions.

Important: Winner and clients are responsible for all Federal, State, and/or Foreign Taxes. Hole-in-WON.com does NOT give tax advice or recommendations. Should a winner or client reside outside the U.S., 35% of award may be withheld for U.S. IRS. Contact an accountant or tax attorney for further advice. Should client or winner have any tax judgment or liens, claim may go towards settlement and balance forwarded. If you will receive an installment/annuity or a car lease award, payment will be made once a year until fulfillment of award according to Hole-in-WON.com installment plan condition document. b) When you have won a trip or prize award: If trip prize was through Hole-in-WON.com you have up to one year AFTER valid claim date to reserve your trip. Check with your tournament director, the event contract and Award Certificate for specific trip details. 90 days minimum is required to confirm your reservations. 120 days if airfare is provided. Blackout dates and restrictions apply, holidays, seasonal prime times not available in most instances. Understand that we are not a travel agency and have no control over airlines and availability of accommodations. These trips are promotional, 2 for 1’s, also the most popular and highest quality destinations available with bookings that will fill quickly. We suggest up to one-year notice to get the times and dates you desire. Supplementary or Consolation Trips have a maximum value of $500.⁰⁰. NO CASH EXCHANGES ALLOWED. Trip exchanges are at the Total Discretion of Hole-in-WON.com. We will make every effort to accommodate all your requests, but once you choose your final dates, NO cancellation and rescheduling will be allowed and/or forfeiture of the trip, (Penalties may be applicable). Once booked, NO exchanges or cancellations will be honored. Trip packages are always subject to change without notice and subject to resort provisions. c)If trip or prize was though an outside 3^rd party company or organization: Hole-in-WON will require verification of the loss, such as a bill of sale for a tangible item. In the case of a trip, prepaid room reservation, itinerary, airline tickets and/or the complete booking for the winner and guests (if applicable).

d)There are NO exceptions. Fraud will be prosecuted to the full extent of the law. Hole-in-WON.com reserves its right at any time to have client indemnify, protect and save harmless Hole-in-WON.com, its directors, employees, or assignees from any costs, expenses, damages, and undue hardship arising from ANY claims or actions by any contestant, director, sponsor, organization, assignee, legal, 3^rd party or otherwise, alleged to have been caused by any act, error, omission, or otherwise, negligent or non-negligent by any party, involved or associated with the event or brought into the process by the direction of the client or not brought in by the client.
